* Patient/Subscriber/Guarantor is responsible for any denied or non-covered services. An estimated co-payment or co-insurance will be collected at the time of the visit. All differences remaining will be billed to the patient. Deductible plans are required to pay the full cost of all visits until the annual deductible amount has been met and verified by the insurance plan.
